I have been having sharp heart cramps coming and going ?

I am a 21 year old female and smoking for 10 years now almost a year ago I started having heart "cramps" pains it bugs me for a few days then goes away for a few days. Sometimes it gets so painfull that I can hardly breath sould I be concerned
